If a family member submits the request on your behalf, they must bring:

• Their own ID card
• A doctor’s letter stating that you are alive, written within 48 hours prior to submission

How to Apply
You must apply in person at:
• A Population and Immigration Authority office in Israel
• An Israeli mission abroad
